OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to struck by, crushing, caught between, and amputation hazards from the unexpected energization of powdered metal and sizing presses during set up operations: A.) Metco Industries Inc. - On or about, 4/22/16, and at times prior thereto, the employer did not develop and implement written die setting procedures for the set-up of the powdered metal presses and sizing presses. Presses included, but were not limited to, Gasbarre Press M35, 400 ton press. Employees perform set up and die changes on the presses.
