29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: a) Crest Bending, Inc. on or about January 29, 2016, at the address of 108 John St. New Washington, OH: The employer failed to ensure that energy control procedures were developed and utilized to control potentially hazardous energy sources on the Colchester Metal Lathe #618 located in the machining department. Employees performing setup and tool adjustment activities in an electrically energized condition were exposed to unexpected startup at or near the machines point of operation.
