29 CFR 1910.212(a)(3)(ii): Point of operation guards were not designed and constructed as to prevent the operator from having any part of their body in the danger zone during the operating cycle: A) On or about December 15, 2022, the employer did not ensure the point of operation on the Price-Thomas Inc. hydraulic press, located on the press floor, was adequately guarded. An employee suffered an amputation injury while attempting to remove a piece of elongated steel that was located inside the tooling die.

29 CFR 1910.305(a)(2)(x): Flexible cords and cables were not protected from accidental damage, as might be caused, for example, by sharp corners, projections, and doorways or other pinch points. A) On or about December 20, 2022, the employer did not ensure that flexible cords and cables were protected from accidental damage. Employees who operate the Price-Thomas hydraulic press are exposed to electrical shock and electrocution hazards. The flexible cord used to supply power to the Price-Thomas hydraulic press is powered directly from the fuse box located adjacently from the hydraulic press.
