29 CFR 1910.106(b)(6): In locations where flammable vapors may be present, precautions were not taken to prevent ignition by eliminating or controlling sources of ignition. Sources of ingition may include open flames, lightning, smoking, hot sirfaces, frictional heat, sparks (static, electrical, and mechanical), spontaneous ignition, chemical and physical-chemical reactions, and radiant heat: Ganther Construction project site located at 2302 E. Glenhurst Ln. Appleton, WI where Ferrellgas Inc. had installed a LP vapor distribution system: On February, 8, 2016, an employee was exposed to burn and explosion hazards when he drove his truck up to a leaking 1000 gallon LP tank and started repairing it. Precautions were not taken and the vehicle's hot exhaust system and engine were potential sources of ignition for the LP vapors.
