29 CFR 1910.23(a)(9): Every floor hole into which persons cannot accidentally walk (on account of fixed machinery, equipment, or walls) shall be protected by a cover that leaves no openings more than 1 inch wide. The cover shall be securely held in place to prevent tools or materials from falling through.    On or about February 9, 2016, the employer exposed employees working at the E2 Adhesive coater, to an unguarded floor opening that measured 35" long by 8 1/4" wide at the moveable table.

29 CFR 1910.106(e)(6)(i): Adequate precautions were not taken to prevent the ignition of flammable vapors:    On or about February 9, 2016, The employer failed to ensure that the SP 366 Degreaser located in the red container, by the north side of the E2 Adhesive coater, used for cleaning various parts was grounded to prevent employee exposure to a fire and explosion hazard.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: Examples of guarding machines are barrier guards, two-hand tripping devices, electronic safety devices, etc:    a.  On or before February 9, 2016, while operating the E2 Adhesive coater located in the west side of the building, the machine operator was exposed to the unguarded E2 laminator.    b.  On or before February 9, 2016, while operating the E2 Adhesive coater located in the west side of the building, the machine operator was exposed to the unguarded rubber roller located on the back end of the coater.
