29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section:    a) Plant - On or about 02/09/16 and at times prior thereto employees performed maintenance/repairs of machinery including (but not limited to) Accupress Press Brake, Standard Press Brake, Fadal CNC, Atlantic Shear. Bridgeport Milling Machine. Richardson Tube Bender, Jet Drill Press, Edwards Iron Worker, Cosen Saw-7652NC, Phoenix Saw PA10, and Cosen Saw SH-500M. The employer did not develop, implement and utilize individual written lockout/tagout procedures to protect employees from moving/rotating parts, electrical energy, and pressurized fluids.    In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.

29 CFR 1910.147(c)(7)(i)(A): Authorized employee(s) did not receive training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation:    a) Plant - On or about 02/09/16 and at times prior thereto employees performed maintenance/repairs of machinery including (but not limited to) Accupress Press Brake, Standard Press Brake, Fadal CNC, Atlantic Shear. Bridgeport Milling Machine. Richardson Tube Bender, Jet Drill Press, Edwards Iron Worker, Cosen Saw-7652NC, Phoenix Saw PA10, and Cosen Saw SH-500M. The employer did not train authorized employees in the recognition of hazardous energy sources, the type and magnitude of the energies available, and the methods and means necessary for energy isolation in each particular machine in order to protect employees from moving/rotating parts, electrical energy, and pressurized fluids.    In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.

29 CFR 1910.212(a)(3)(ii): Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s):    a) Plant - On or about 02/09/16 and at times prior thereto employees operated the Atlantic hydraulic shear Model #HDE, S.N. #801462 to cut sheet metal. The point of operation was not guarded, exposing employees to amputation hazards.    b) Plant - On or about 02/09/16 and at times prior thereto employees operated the Standard hydraulic press brake to bend sheet metal. The point of operation was not guarded, exposing employees to amputation hazards.    c) Plant - On or about 02/09/16 and at times prior thereto employees operated the Standard hydraulic press brake to bend sheet metal. The point of operation was not guarded in the back of the machine, exposing employees to amputation hazards.    Or in the Alternative:    29 CFR 1910.212(a)(1) One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, and rotating parts:    a) Plant - On or about 02/09/16 and at times prior thereto employees operated the Atlantic hydraulic shear Model #HDE, S.N. #801462 to cut sheet metal. The point of operation was not guarded, exposing employees to amputation hazards.    b) Plant - On or about 02/09/16 and at times prior thereto employees operated the Standard hydraulic press brake to bend sheet metal. The point of operation was not guarded, exposing employees to amputation hazards.    c) Plant - On or about 02/09/16 and at times prior thereto employees operated the Standard hydraulic press brake to bend sheet metal. The back of the machine was not guarded, exposing employees to amputation hazards.
