29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents. On or about February 10, 2016 and at times prior thereto, employees were using pneumatic nail guns and hammers without eye protection. This condition exposed employees to eye injuries.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards. On or about February 10, 2016 and at times prior thereto, at the worksite located at or near 509 Pleasant Valley Drive, Little Rock, Arkansas, employees were routinely exposed to fall hazards without having training in fall hazards. This condition exposed employees to fall hazards.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employees provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b). This employer does not protect employees from fall hazards. This violation was observed on or about February 10, 2016 at 509 Pleasant Valley Drive, Little Rock, Arkansas, where employees were working at heights greater than 6 feet while performing roofing work without fall protection. Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the date of this citation, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure that employees are protected from falls while performing roofing work. Angelo Roofing was previously cited for a repeat violation of this occupational safety and health standard or its equivalent standard CFR 1926.501(b)(11), which was contained in OSHA inspection number 1061614, citation number 1, item number 1 and was affirmed as a final order on June 23, 2015, with respect to a workplace located at 2501-2617 S. Main Street, Little Rock, Arkansas.
