This was most recently documented as having occurred on February 15, 2016, at the Stahl Specialty Company facility located at 1301 Stahl Drive in Warrensburg, Missouri, where:    (a) employees were performing maintenance on the Laempe LFB50_H Sand Core Machine (#5527) without an authorized person installing a safety pin in its proper (uppermost) position to arrest any potential downward movement of the ram during the maintenance activity, and    (b) an employee was performing normal production operations clean-outs of the Core Box on the second Laempe Sand Core Machine, Model LB50_H, Serial No. 110741 which required him to physically enter the core box without lockout / tagout devices being applied by an authorized person to isolate the machine from multiple energy sources including electrical, hydraulic, and pneumatic sources.
