OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that the employer did not provide training for the proper use of the dock leveling device at the facility. On or about February 16, 2016, at the loading dock, employees were exposed to crushing and amputation injuries when adjusting the position of the lip plate of the dock leveling extension by hand.

OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to amputation hazards where the employer did not maintain cautions and instructions markings for the operation and safe use of the dock leveling device at the facility. On or about February 16, 2016, at the loading dock, cautions and instruction markings for the operations and safe use of a dock leveling device were not maintained by the employer. An employee suffered the amputation of the right ring fingertip while operating the dock leveling device.
