29 CFR 1915.73(b): When employees were working in the vicinity of flush manholes and other small openings of comparable size in the deck and other working surfaces, such openings were not suitably covered or guarded to a height of not less than 30 inches. A) At dry dock # 9 where an employee working on the wingwall deck fell approximately 20-25 feet to the safety deck below after stepping on a wooden cover that was covering an opening on or about 02/15/2016.
