29 CFR 1926.451(b)(5)(ii): The end of a platform greater than 10 feet in length extended over its support more than 18 inches and the platform was not designed and installed so that the cantilevered portion of the platform is able to support employees and/or materials without tipping, nor did it have guardrails which block employee access to the cantilevered end:   a) Jobsite, front of house, 174 Franklin Avenue, Vandergrift, PA 15690:  Observed on or about February 25, 2016, each end of a Werner brand scaffold stage (manufacturer number 2024) located on the roof of the front porch extended over its supports more than 18 inches.  The right end of the scaffold stage extended approximately 28 inches over its support.  The left end of the scaffold stage extended approximately 48 inches over its support.  An employee who was working from the scaffold was exposed to a fall hazard of approximately 19 feet to the ground below.    b) Jobsite, west side of house, 174 Franklin Avenue, Vandergrift, PA 15690:  Observed on or about February 25, 2016, each end of a Werner brand scaffold stage (manufacturer number 2024) located on the west side of the house extended over its supports more than 18 inches.  The right end of the scaffold plank extended approximately 42 inches over its support.  The left end of the scaffold stage extended approximately 30 inches over its support.  Employees who were working from the scaffold were exposed to a fall hazard of approximately 22 feet to the ground below.

29 CFR 1926.451(e)(1):  When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from another scaffold, structure, personnel hoist, or similar surface was not used:  a) Jobsite, front of house, 174 Franklin Avenue, Vandergrift, PA 15690:  Observed on or about February 25, 2016, a ladder jack scaffold located above the front porch roof was not equipped with a safe means of access to the stage.  Employees were exposed to a fall hazard of approximately 19 feet to the ground below while accessing the stage.  a) Jobsite, west side of house, 174 Franklin Avenue, Vandergrift, PA 15690:  Observed on or about February 25, 2016, a ladder jack scaffold located on the west side of the house was not equipped with a safe means of access to the stage.  Employees were exposed to a fall hazard of approximately 22 feet to the ground below while accessing the stage.

The employee was exposed to a fall hazard of up to approximately 19 feet to the ground below.   b) Jobsite, 174 Franklin Avenue, Vandergrift, PA 15690:  Observed on or about February 25, 2016, an employee working off of a ladder jack scaffold located on the west side of the house were not protected from falling.  The employee was exposed to a fall hazard of approximately 22 feet to the ground below.

An employee working on the scaffold was exposed to a fall hazard of approximately 19 feet to the ground below.

29 CFR 1926.451(f)(6):  Scaffolds were erected, used, dismantled, altered, or moved such that they, or any conductive material handled on them, could have come closer than the minimum distances established for specific voltages as set forth in the table in subparagraph (f)(6):   a) Jobsite, 174 Franklin Avenue, Vandergrift, PA 15690:  Observed on or about February 25, 2016, an energized triplex service drop with a single-phase voltage of 240 Volts AC was laying on a Werner brand aluminum scaffold stage (manufacturer number 2024).  Employees were exposed to an electrocution hazard while using the stage to access the roof.

Employees were exposed to fall hazards of up to approximately 22 feet while working off of ladder jack scaffolds that were constructed with multiple deficiencies.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501(b):    a)  Jobsite, 174 Franklin Avenue, Vandergrift, PA 15690:  Observed on or about February 25, 2016, employees egaged in roofing activities on a multi story residential dwelling with a sloped roof were not protected from falling by the use of a guardrail system, safety net system, or personal fall arrest system.  The employees were exposed to a fall hazard of approximately 22 feet to the ground below.  The slope of the roof was approximately 10:12 (vertical to horizontal).   Chuck Musta dba Chuck Musta was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1061029, citation number 1, item number 1(a) and was affirmed as a final order on June 19, 2015, with respect to a workplace located at Grand Avenue, Unit 27, Neville Island, PA 15225.

The ladder side rails did not extend at least 3 feet (0.9 m) above the upper landing surface.  Employees using the ladder were exposed to a fall hazard of approximately 13 feet to the ground below.  Chuck Musta dba Chuck Musta was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.1053(b)(1), which was contained in OSHA inspection number 1061029, citation number 1, item number 1(a) and was affirmed as a final order on June 19, 2015, with respect to a workplace located at Grand Avenue, Unit 27, Neville Island, PA 15225.
