29 CFR 1910.147(e)(1): Before lockout or tagout devices were removed and energy was restored to the machine or equipment, authorized employees did not inspect the work area to ensure that nonessential items had been removed and that machine or equipment components are operationally intact: (a) Board Line: On February 21, 2016, the Board Line was locked out for maintenance and servicing and had the chain and sprocket guard removed. The employer did not ensure that the guard was replaced on the Board Line before lockout devices were removed and energy was restored to the machine.
