29 CFR 1910.212(a)(3)(ii): Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s): a) On or about January 13, 2020 and times prior, employees used a block cut off saw to trim boards used for making pallets. The point of operation was not adequately guarded in that the guard on the block cut off saw had a gap, up to 4 1/2 inches, between the bottom of the hood and the material being cut.

29 CFR 1904.39(a)(2): The employer did not report within 24-hours a work-related incident resulting in in-patient hospitalization, amputation or the loss of an eye. a) On or about January 13, 2020 an employee was injured in a work related incident when his hand was pulled into an operating block saw. The injury required inpatient hospitalization and resulted in an amputation which were not reported to OSHA within 24 hours.
