29 CFR 1904.39(a)(2):The employer did not report within 24-hours a work-related incident resulting in in-patient hospitalization, amputation or the loss of an eye. a) 442 Harland Ritter Road: On or about July 11, 2024, an employee received an amputation injury that required inpatient hospitalization. The employer did not report the amputation to OSHA within 24-hours. Lumber and Things, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29CFR 1904.39(a)(2) which was contained in OSHA inspection number 1466035, citation number 2, item number 1 and was affirmed as a final order on January 18, 2021 with respect to a workplace located at 825 Keyser Industrial Park, Keyser, WV 26726.
