29 CFR 1910.22(b)(1):  Where mechanical equipment was used, aisles and passageways were not kept clear and in good repair, with no obstruction across or in aisles that could create a hazard:  A.  On or about March 8, 2016, Warehouse:  Where the Big Joe reach truck was used near the bay door, the concrete floor was damaged. Half-inch deep holes measuring eight to ten inches across were filled with sand and covered with cardboard and a rubber mat.  The mat was not secured and shifted during equipment operation.

29 CFR 1910.37(a)(4): Safeguard(s) designed to protect employees during an emergency (e.g., sprinkler systems, alarm systems, fire doors, exit lighting), were not in proper working order at all times:  A.  On or about March 8, 2016, Warehouse:  In the following locations, safeguards on the emergency exit doors were not in proper working order.  1.  Emergency exit door near the baler equipment:  The self-retracting arm on the door was broken, allowing the door to fully open.  2.  Emergency exit door near the outdoor compactor equipment:  The self-retracting arm on the door was missing, allowing the door to fully open.

29 CFR 1910.178(p)(1): Powered industrial truck(s) found to be in need of repair, defective, or in any way unsafe had not been  taken out of service until restored to safe operating condition(s):  A.  On or about February 14, 2016, Warehouse:  The Big Joe lift truck, Model PDC-25-168-TR, S/N 4410057, had a hydraulic leak identified by operators.  The vehicle had not been taken out of service and the hazard had not been addressed by the time of the OSHA inspection on March 8, 2016.

On or about March 8, 2016, Warehouse:  Employees required to pour water into the forklift battery for the Big Joe lift truck, Model PDC-25-168-TR, were not provided with an apron or other frontal torso protection to prevent being splashed with battery acid.  Savers, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard (1910.132(a)), which was contained in OSHA inspection number 9365365, citation number 1, item number 4a and was affirmed as a final order on November 27, 2013, with respect to a workplace located at Store 1163, 495 Chamberlain Highway, Meriden, CT  06451.
