29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents:  (a)  On or about March 3, 2016, employees were exposed to eye injuries resulting from utilizing pneumatic nailers while performing framing activities on a residential building without the benefit of eye protection.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):    (a)  On or about March 3, 2016, employees were exposed to injuries from falls, while conducting framing activities from an unfinished second floor, in excess of 6 feet above the lower working level, without the benefit of fall protection systems being in place.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  (a)  On or about March 3, 2016, employees were exposed to injuries resulting from falls, while accessing the second story floor from a portable ladder that did not extend at least 3 feet above the upper landing surface being accessed.
