29 CFR 1910.23(c)(1): Open-sided floors and/or platforms four feet or more above adjacent floor or ground level were not guarded with standard railings: a) Production floor area; An employee was performing maintenance on top of the BOBST Autoplaten Die Cutter without the use of fall protection. The employee was exposed to a fall height between 6 feet 11 inches and 8 feet 7 inches, on or about 02/22/2016. Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement. Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic or video evidence of abatement or other written records.

29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: a) Production floor area; An employee climbed onto the BOBST Autoplaten Die Cutter to clear a jam without locking out the machine. The machine cycled striking and fracturing the employee's femur, on or about 02/22/2016. Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement. Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic or video evidence of abatement or other written records.
