29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):       a.   Employees performing roofing activities on a new two-story residence, are not protected by any form of fall protection systems, exposing them to a 40-foot fall to the ground level parking lot and driveway below.      The employer fails to ensure effective protection against falls as required by the standard, as demonstrated on March 14, 2016, when employees installing a shingles on the 5:12 pitched roof of a house, located at 28616 Grand View Manor in Yulee, Florida, were exposed to 10-foot fall hazards above lower levels in that they did not have any form of fall protection systems in place at the time of the hazard exposure.       Coastal Building Systems of Amelia, Inc. was previously cited for a Repeat violation of this occupational safety and health standard or its equivalent standard (1926.501(b)(13)), which was contained in OSHA inspection number 581200, citation number 1, item number 1 and was affirmed as a final order on December 4, 2012, with respect to a workplace located at 279 St. Croix Island Dr., Saint Augustine, Florida 32092.       Coastal Building Systems of Amelia, Inc. was previously cited for a Willful violation of this occupational safety and health standard or its equivalent standard (1926.501(b)(13)), which was contained in OSHA inspection number 581138, citation number 1, item number 1 and was affirmed as a final order on December 4, 2012, with respect to a workplace located at 279 St. Croix Island Dr., Saint Augustine, Florida 32092.
