29 CFR 1910.24(f): All treads shall be reasonably slip-resistant and the nosings shall be of nonslip finish. Welded bar grating treads without nosings are acceptable providing the leading edge can be readily identified by personnel descending the stairway and provided the tread is serrated or is of definite nonslip design. Rise height and tread width shall be uniform throughout any flight of stairs including any foundation structure used as one or more treads of the stairs:    On or about March 14, 2016, the employer did not ensure that the left "outbound body wash" stairway was installed with uniform rise height and reasonably slip resistant treads prior to authorizing cleaning of the elevated area.  Employees accessing the area by way of these stairs were exposed to falls while carrying waste material (grease and sludge) in buckets.      Abatement certification is required for this item in accordance with the requirements of 29 CFR 1913.19(c).

29 CFR 1910.24(h): Standard railings shall be provided on the open sides of all exposed stairways and stair platforms. Handrails shall be provided on at least one side of closed stairways preferably on the right side descending. Stair railings and handrails shall be installed in accordance with the provisions of 1910.23:     On or about March 14, 2016, the employer did not ensure installation of adequate handrails for the entire length of stairway to the "outbound body wash" in accordance with 1910.23. Only one side was protected by a handrail and the top of its upper rail measured only 25.5 inches vertical height from the tread.  Employees were exposed to falls while accessing the area through the stairway to remove excess waste (grease and sludge) in buckets.     Abatement certification is required for this item in accordance with the requirements of 29 CFR 1903.19(c).

29 CFR 1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6):    a) On or about February 22, 2016 and thereafter, employees exposed to struck-by and burn hazards from release of hazardous energy and unexpected startup were not required to perform the following sequence of actions prior to removing and replacing filters (servicing) within burner boxes under the paint line and draining/removing flood water (servicing) within a robot cell in the assembly area:     1) Machine or equipment shutdown.   2) Machine or equipment isolation.   3) Lockout or tagout device application.  4) Verification of isolation.    Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR 1910.147(c)(7)(i): The employer did not provide adequate training to ensure that employees acquired the knowledge and skills required for the safe application, usage and removal of energy control devices:   On or about February 22, 2016, and thereafter, the employer did not ensure that authorized employees servicing within robot cells  and burner boxes  had acquired the requisite knowledge and skills to for the safely apply and use energy control devices to isolate energy to industrial robots, automated assembly equipment and drying ovens prior to commencing servicing activities.  Employees were exposed to burns and struck-by hazards.   Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR 1910.146(c)(1): The employer did not evaluate the workplace to determine if any spaces were permit-required confined spaces:  On or about January 23, 2016 and thereafter, the employer failed to conduct an evaluation of all spaces accessed by Tri-Dim Filter Corp employees to determine if any spaces were permit-required  confined spaces. The following spaces had not been evaluated. Employees were exposed to being trapped in fan houses and paint line burner boxes while servicing filter assemblies when unexpectedly started by the host employer.   Abatement certification is required for this item in accordance with the requirements of 29 CFR 1903.19(c).
