29 CFR 1910.133(a)(1): The employer did not ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation: a) Polishing area: On or about March 24, 2016, the employer did not ensure the use of eye protection, such as, but not limited to safety glasses, for employee(s) who were exposed to eye hazards from flying particles and silica dust, which were generated during the use of a Makita 6-inch Angle Grinder (Model SJSII, SN: 41452A) for granite dry polishing and grinding processes. b) Polishing area: On or about March 24, 2016, the employer did not ensure the use of eye protection, such as, but not limited to safety glasses, for employee(s) who were exposed to eye hazards from flying particles and silica dust, which were generated during the use of a Makita 6-inch Angle Grinder (Model SJSII, SN: 41141A) for granite dry polishing and grinding processes. c) Polishing area: On or about March 24, 2016, the employer did not ensure the use of eye protection, such as, but not limited to safety glasses, for employee(s) who were exposed to eye hazards from flying particles and silica dust, which were generated during the use of a Makita 6-inch Angle Grinder (Model SJSII, SN: 225436A) for granite dry polishing and grinding processes. The STI Granite & Marble, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.243(c)(1)(ii)(a), which was contained in OSHA inspection number 1057795, citation number 1, item number 1 and was affirmed as a final order on 09/09/15 with respect to a workplace located at 300 W.
