29 CFR 1910.134(c): The employer did not develop and implement a written respiratory protection program with required worksite-specific procedures and elements for required respirator use: Polishing area: On or about April 20, 2016, and times prior thereto, the employer did not establish a written respiratory protection program or implement provisions of the program, such as but not limited to medical evaluations or selection and maintenance of respirators, for employees who were required to wear 3M N-95 filtering face piece respirators and were exposed to inhalation hazards in work areas where exposures to respirable crystalline silica were found to exceed the permissible exposure limit for an eight hour work shift. STI Granite and Marble, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard, which was contained in OSHA inspection number 1057795 citation number 1, item number 2 and was affirmed as a final order on October 1, 2015, with respect to a workplace located at 300 W. Rhapsody, San Antonio, TX 78216.

29 CFR 1910.1000(c): An employee(s) was exposed to respirable crystalline silica quartz in excess of the 8-hour time weighted average limits of (10 mg/m^3)/ (%SiO2+2 )listed in Table Z-3: a) Polishing area: On or about April 20, 2016, an employee performing dry polishing operations to granite slabs using a Makita 6-inch Angle Grinder (Model 9566CV, SN: 41141A) was exposed to respirable dust containing 11% crystalline silica (quartz) at an 8-hour Time Weighted Average of 1.55 mg/m^3, approximately 2.02 times the Permissible Exposure Limit of 0.769 mg/m^3 during a 310 minute sampling period. Sample calculations include an assumed zero exposure for the 170 minutes not sampled. b) Polishing area: On or about April 20, 2016, an employee performing dry polishing operations to granite slabs using a Makita 6-inch Angle Grinder (Model 9566CV, SN: 41102A) was exposed to respirable dust containing 17% crystalline silica (quartz) at an 8-hour Time Weighted Average of 1.50 mg/m^3, approximately 2.85 times the Permissible Exposure Limit of 0.526 mg/m^3 during a 313 minute sampling period. Sample calculations include an assumed zero exposure for the 167 minutes not sampled. STI Granite and Marble, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard, which was contained in OSHA inspection number 1057795 citation number 1, item number 5a and was affirmed as a final order on October 1, 2015, with respect to a workplace located at 300 W.
