Park Avenue, Saint Louis, MO 63110, on or about 3/28/2016, located in the in A Bay South welding section, An Aronson Manipulator welding unit, Model: 1C04MR04CL-MKKT-B0XED M4ST, S/N: 8190, live parts of an electric motor operating at 50 volts or more were not guarded against accidental contact by use of approved cabinets. The worker was exposed to a potential electrical shock, burn, or electrocution injury.

Park Avenue, Saint Louis, MO 63110, on or about 3/28/2016, in the A Bay South Welding section, the power cord of an Aronson Manipulator welding unit, Model: 1C04MR04CL-MKKT-B0XED M4ST, S/N: 8190, the power tap was not connected so that strain relief would prevent pull from being directly transmitted to joints or terminal screws.
