29 CFR 1926.503(a)(2)(viii): The employer did not assure that each employee had been trained by a competent person qualified in the standards contained in subpart M: a) On or about March 28, 2016 at W6415 Sonny Drive in Menasha, employer failed to provide training by a competent person for employees who did not recognize fall hazards associated with walking on a roof surface at 18.5 ft. above surface level without utilizing fall protection equipment. Employees were not protected during cleaning-up of debris and equipment on the roof surface.
