29 CFR 1926.300(b)(2):  Belts, gears, shafts, pulleys, sprockets, spindles, drums, fly wheels, chains, or other reciprocating, rotating or moving parts of equipment were not guarded when such parts were exposed to contact by employees or otherwise created a hazard:  (a)  On or about April 7, 2016, during nailing activities on the Jenny Air Compressor, Model #KU, Serial# F051812208, located at a site at 3550 French Rd, Little Chute, WI 54913, the employer did not ensure that machine guarding was provided over the belt and pulley.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   a)  On or about April 7, 2016, during roofing work at a site located at 3550 French Rd, Little Chute, WI 54913, the employer failed to ensure that employees used a fall protection system when exposed to falls greater than 16 feet to the cement floor from a high slope roof.
