29 CFR 1910.212(a)(3)(ii): Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s):   (a) Southeast Corner of Shop/Cincinnati Shear: On or about 3/29/2016, the shear was not equipped with an adequate guard to prevent fingers from entering the point of operation.  (b) Southeast Corner of Shop/Cincinnati Shear: On or about 3/29/2016, the shear's foot treadle bar was not equipped with a guard and/or protective devices to prevent the unintentional activation of the machine while it is in operation.
