29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b): a) On or about March 22, 2016; Second floor of the residential home (Southside); employees were exposed to an estimated fall of 12 foot fall to ground level while framing the residential home; employees were working near the edge during the process. b) On or about March 22, 2016; Second floor of the residential home; employees were exposed to 10-15 foot falls to ground level while working near floor openings within the second floor area. In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the Certification of Corrective Action Worksheet).

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed a) On or about March 22, 2016; second floor of the residential home; employees were using an extension ladder to access the second floor of the project. The extension ladder was 18 inches above the platform exposing them to a potential fall hazard of ten feet to the lower level. In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the Certification of Corrective Action Worksheet).
