29 CFR 1926.350(a)(10): Oxygen cylinders in storage were not separated from fuel-gas cylinder or combustible materials (especially oil or grease), a minimum distance of 20 feet or by a noncombustible barrier at least 5 feet high having a fire-resistance rating of at least one-half hour. Excavation Site: On or about March 31, 2016 and at times prior thereto; the employer was storing oxygen and acetylene compressed gas cylinder/bottles without a barrier. This condition exposed the employees to burn and respiratory hazards.

Excavation Site: On or about March 31, 2016, and at times prior thereto, two employees were working on the edge of excavation without any form of fall protection. This condition exposed the employees to a fall greater than 6-feet to the ground below.
