29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from another scaffold, structure, personnel hoist, or similar surface were not used. Cross braces shall not be used as a means of access:    At a new residential construction site, employees were not provided with a ladder for access and/or egress to and from the upper working levels of the scaffold and climbed the end frames exposed to the hazard of falling approximately 14-feet to the lower level.

29 CFR 1926.102(a)(1): Employees shall be provided with eye and face protection equipment when machines or operations present potential eye or face injury from physical, chemical, or radiation agents.   At a new residential construction site, employees operating pneumatic nail guns and circular saws were not protected from eye injuries by wearing protective eyewear.     Victor Paz was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.102(a)(1), which was contained in OSHA inspection number 316094457, Citation Number 1, Item number 1 and was affirmed as a final order on May 22, 2012, with a final abatement date of  May 23, 2012 with respect to a workplace located at 688 Arbol, Irving, Texas 75039

At a new residential construction site, employees performing framing activities from a Ladder Jack scaffold system that was 14 inches wide were exposed to fall hazards.   Victor Paz was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.451(b)(2), which was contained in OSHA inspection number 1048674, Citation Number 1, Item number 1 and was affirmed as a final order on July 10, 2015,  with a final abatement date of  July 10, 2015 with respect to a workplace located at 2408 Barclay Court, Plano, Texas 75074.
