29 CFR 1926.452(j)(1): Pump jack brackets, braces, and accessories shall be fabricated from metal plates and angles. Each pump jack bracket shall have two positive gripping mechanisms to prevent any failure or slippage. On or about August 30, 2019 employees were exposed to falls while working on pump jack scaffold approximately 14 feet to ground level by not installing adequate metal brackets or braces on poles of pump jack.

29 CFR 1926.451(g)(1): Each employee on a scaffold more than 10 feet (3.1 m) above a lower level was not protected from falling to that lower level. Paragraphs (g)(1)(i) through (vii) of this section establish the types of fall protection to be provided to the employees on each type of scaffold. Paragraph (g)(2) of this section addresses fall protection for scaffold erectors and dismantlers. Note to paragraph (g)(1): The fall protection requirements for employees installing suspension scaffold support systems on floors, roofs, and other elevated surfaces are set forth in subpart M of this part. On or about August 30, 2019 employees where expose to fall hazards of approximately 14 feet to lower ground level while working on the pump jack scaffold without fall protection. The Victor Paz was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1026.451(g)(1), which was contained in OSHA inspection number 141017, citation number 1, item number 1 and was affirmed as a final order on 08/06/2019, with respect to a workplace located at 3519 Calico Dr. Irving, TX 75061 The Victor Paz was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1026.451(g)(1), which was contained in OSHA inspection number 1137012, citation number 2, item number 3 and was affirmed as a final order on 07/11/2016, with respect to a workplace located at 8129 Whistling Duck Drive, Fort Worth, TX 76118. The Victor Paz was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1026.451(g)(1), which was contained in OSHA inspection number 1048674, citation number 2, item number 1 and was affirmed as a final order on 07/10/2015, with respect to a workplace located at 2408 Barlay Court, Plano, TX 75074.
