29 CFR 1926.453(b)(2)(v): A body belt was not worn and/or a lanyard was not attached to the boom or basket when working from an aerial lift:    (a)  304 South Water Street, New Oxford Pennsylvania - Employees used a Genie aerial lift to reach a roof.  The employer did not ensure that each employee was provided and used a harness and lanyard when traveling in the basket, on or about April 5, 2016.      The Waggoner Construction Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.453(b)(2)(v), which was contained in OSHA inspection number 315559682, citation number 1, item number 2 and was affirmed as a final order on July 8, 2011, with respect to a workplace located at 340 Stoney Hill Road, Greensboro Pennsylvania 15338.     No abatement certification or documentation required.

29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system.  Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system.    (a) 304 South Water Street, New Oxford Pennsylvania - Employees worked installing a rubber roof.  The employer did not ensure that employees exposed to a 14 foot fall hazard were provided with and used fall protection, on or about April 5, 2016.    The Waggoner Construction Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(10), which was contained in OSHA inspection number 976542, citation number 1, item number 1 and was affirmed as a final order on June 18, 2014, with respect to a workplace located at West Elm Avenue, Hanover Pennsylvania.    The Waggoner Construction Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(10), which was contained in OSHA inspection number 315559682, citation number 1, item number 2 and was affirmed as a final order on July 8, 2011, with respect to a workplace located at 340 Stoney Hill Road, Greensboro Pennsylvania.     No abatement certification or documentation required.
