(a) Generator/Air compressor Bldg:  On or about 6 April 2016 and times prior thereto, the electrical panel P-227 was not properly identified with a listing for the circuits.  This condition exposed employees to an electrical shock and burn hazard.    (b) Docking Shack:  On or about 6 April 2016 and times prior thereto, the electrical panel (number unknown) was not properly identified with a listing for the circuits.  This condition exposed employees to an electrical shock and burn hazard.

Machine shop:  On or about 6 April 2016 and times prior thereto, the 480 volt Bus ducts had four locally fabricated wooden end caps installed and duct taped into place. Machine Shop:  On or about 6 April 2016 and times prior thereto, the work rest measured at 3/8 inch.  This exposed employees to struck-by hazard from work piece getting stuck between rest and wheel causing wheel to break.
