29 CFR 1915.71(d)(4): Posts of independent Pole metal Scaffolds were not fitted with bases supported on a firm foundation to distribute the load. USCG M/V Mustang, Vigor Alaska Shipyard: On or about 6 April 2016 and times prior thereto, the secondary exit scaffolding, constructed of independent pole metal, measured at approximately 18 feet tall, did not have a firm foundation to distribute weight. This condition exposed employees to an fall hazard.
