29 CFR 1910.219(c)(2)(i): All exposed part(s) of horizontal shafting seven (7) feet or less from floor or working platform were not protected by stationary casing(s) enclosing shafting completely or by trough(s) enclosing sides and top or sides and bottom of shafting:   (a)  Machine Shop:  On or about April 4, 2016 and at times prior thereto, the horizontal transmission shaft operating five (5) feet above the floor surface on the Cincinnati Bickford "The Super Service Radial" Drill Press (ID#003674) was not guarded.  This condition exposed employees to caught-in hazards.   (b)  Machine Shop:  The horizontal transmission shaft operating 30 inches above the floor surface on the G&L Horizontal Mill was not guarded.  This condition exposed employees to caught-in hazards.   General Industry standard applicable to shipyard employment.

29 CFR 1910.219(c)(3): Vertical or inclined shafting seven -7 feet or less from floor or working platform was not enclosed with stationary casing:   Machine Shop:  On or about April 4, 2016  and at times prior thereto, the vertical transmission shaft operating 30 inches above the floor surface, 32 diagonal inches from the controls, and 20 inches on the G&L Horizontal Mill was not guarded.  This condition exposed employees to caught-in hazards.  General Industry standard applicable to shipyard employment.

29 CFR 1910.213(i)(1): Non-working portion(s) of the blade of bandsaw(s) were not enclosed or guarded:  Fab Shop:  On or about April 4, 2016 and at times prior thereto, a minimum 10 inches of the non-working portion of the blade on the Powermatic Model No. 143, Serial No. 8643153 band saw was not guarded or enclosed.  This condition exposed employees to struck-by hazards.  General Industry Standard applicable to Shipyard employment.

29 CFR 1910.253(b)(4)(iii): Oxygen cylinders in storage were not separated from fuel-gas cylinders or combustible materials (especially oil or grease), a minimum distance of 20 feet (6.1 m) or by a noncombustible barrier at least 5 feet (1.5 m) high having a fire-resistance rating of at least one-half hour:   Machine Shop:  On or about April 4, 2016 and at times prior there to, two (2) oxygen cylinders were stored 42 inches away from two (2) reserve cylinders of acetylene.  This condition exposed employees to smoke-inhalation hazards from rapid acceleration of fire.     General Industry Standard applicable to Shipyard work.
