29 CFR 1926.300(a): Hand tool(s), power tool(s) or similar equipment were not maintained in a safe condition: (a) Jobsite, Allegheny General MRI Dept., Pittsburgh, PA: On or about April 7, 2016 the Makita 6" right angle grinder being used with a metal cutting wheel was not maintained in a safe condition in that it did not have the side grip installed as required by the manufacturer when in use. An employee was using the grinder to cut metal door frames and held the tool by placing both hands on the shaft. The employee sustained a severe laceration to their left bicep when the tool kicked back and struck the employee.
