29 CFR 1910.179(l)(3)(i): Unsafe condition(s) disclosed during the frequent or periodic inspection(s) required by 29 CFR 1910.179(j) were not corrected before operation of the crane(s) was resumed: A.) Western Pennsylvania Steel Fabricating, Inc. - On or about 4/20/16, and at times prior thereto, cranes were operated with unsafe conditions. Cranes included, but were not limited to, Whitting #9 crane. The crane's bridge brakes were defective in that the bridge had drift after the button was released on the pendant. In addition, the pendant to operate the crane was shutting off randomly.

Western Pennsylvania Steel Fabricating, Inc. - On or about 4/20/16, and at times prior thereto, caution signs were not affixed to and/or in the spray painting area of the Southern Bay to caution personnel against the hazards of welding and smoking in the area while spray painting operations were taking place. Employees spray flammable material including, but not limited to, Corothane, I-MIO-Zinc Moisture Cure Urethane.
