Western Pennsylvania Steel Fabricating, Inc., 550 Honeybee Lane, New Castle, PA 16105, on or about March 4, 2020 and times prior thereto:  Employees were exposed to a struck-by/entrapment hazard while operating the Taylor Y-52-WOM forklift (S/N:  S-70-12651) when performing work operations that included, but were not limited to, moving steel parts and fitting tables.  The forklift truck was not equipped with an operator restraint device (seatbelt).

29 CFR 1910.147(c)(4)(ii): Procedures did not clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, and the means to enforce compliance including, but not limited to, 29 CFR 1910.147(c)(4)(ii)(A), (c)(4)(ii)(B), (c)(4)(ii)(C) and (c)(4)(ii)(D):    A.)  Western Pennsylvania Steel Fabricating, Inc., 550 Honeybee Lane, New Castle, PA 16105, on or about March 4, 2020 and times prior thereto:  Employees were performing service and maintenance activities on equipment including, but not limited to, a Taylor Y-52-WOM forklift (S/N:  S-70-12651).  The specific lockout/tagout procedure for the Taylor Forklift stated to tie a red tag to the steering wheel, and does not state to apply a tagout tag to the energy-isolating device (battery cable / battery terminal).    B.)  Western Pennsylvania Steel Fabricating, Inc., 550 Honeybee Lane, New Castle, PA 16105, on or about March 4, 2020 and times prior thereto:  Employees were performing service and maintenance activities on equipment including, but not limited to, a Taylor Y-52-WOM forklift (S/N:  S-70-12651).
