29 CFR 1910.147(c)(4)(ii): The energy control procedures did not clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, including, but not limited to Items of this section: A.) Western Pennsylvania Steel Fabricating Inc. - On or about 4/20/16, and at times prior thereto, employees were performing servicing and maintenance on machinery that included, but was not limited to, a thermal stress reliever, a hydraulic press brake, and a hydraulic press. The company's lockout/tagout procedures were deficient in that they did not include items including, but not limited to, magnitudes of energy, specific procedural steps for shutting down machinery, a step to verify deenergization of equipment, methods to block/bleed energy, and specific procedural steps for restoring the equipment to full energy.
