29 CFR 1910.37(a)(3): Exit route(s) were not kept free and unobstructed: a. In the Northwest corner of the shop area - On April 27, 2016, the exit route to the emergency exit in the Northwest corner of the shop was partially blocked with stacks of pallets. b. In the center of the shop - On April 27, 2016, one of the two pallet disassembly saws (the one on the West side) was completely surrounded on three sides by stacks of pallets and on one side by the trim saw and associated wood debris and scrap, blocking emergency egress for the two workers operating the pallet disassembly saw.

29 CFR 1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6): (a) Unidentified red chops saw #3 - On or about April 20, 2016 a worker received an amputation injury to a finger while attempting to clear a jam from the saw. The employer failed to implement energy control application steps as the saw was not shut down or turned off to perform the servicing work [per the 1910.147(d)(2) requirements]. As a result, the remaining applicable energy control elements, involving saw isolation [(d)(3)], LOTO device application [(d)(4)], dissipation of residual energy [(d)(5)(i)], and verification of isolation [(d)(6)], were not implemented to protect employees from saw servicing hazards.

29 CFR 1910.213(g)(1): Sliding cutoff saw(s) were not provided with an automatically adjusting hood that completely enclosed the upper half of the saw, the arbor end, and the point of operation at all positions of the saw: (a) North end of the shop - On or about April 20, 2016, the unmarked chop saw #3 was not fully guarded. The guarding device that had been added after the previous inspection to the in-feed side of the saw's upper hood had been damaged and removed exposing workers to laceration and amputation hazards. Ash Pallet Management, Inc. was previously cited for a violation of this Occupational Safety and Health Standard 29 CFR 1910.213(g)(1), which was contained in OSHA inspection number 990043, citation number 1, item number 3 and was affirmed as a final order on October 16, 2014, with respect to a workplace located at 3145 Central Ave., Waukegan, IL 60085.
