29 CFR 1926.451(b)(1): Each platform on all working levels of scaffolds was not fully planked or decked between the front uprights and the guardrail supports as specified in paragraphs 1926.451(b)(1)(i)-(ii): Worksite located at 18715 E Admiral Pl. Catoosa, OK 74015. On or about April 28, 2016, employees were observed working from multiple levels of the scaffolding that were not fully planked between the upright and guardrail supports. Employees were exposed to a fall hazard of more than 10 feet.

29 CFR 1926.451(g)(1): Employees on scaffolds more than 10 feet (3.1 m) above a lower level were not protected from falling to that lower level by fall protection established in paragraphs (g)(1)(i)-(vii) of this section: Worksite located at 18715 E Admiral Pl. Catoosa, OK 74015. On or about April 28, 2016 an employee was observed working from the top level of the scaffolding and was not protected from falling to the lower level by fall protection. The employee was exposed to a fall hazard of more than 30 feet.

29 CFR 1926.451(g)(4)(iii): When midrails, screens, mesh, intermediate vertical members, solid panels, or equivalent structural members are used, they were not installed between the top edge of the guardrail system and the scaffold platform: Worksite located at 18715 E Admiral Pl. Catoosa, OK 74015. On or about April 28, 2016 cross braces were used as the top rail, but no mid rail was provided on the fabricated frame scaffolding. The employees working from the scaffolding were exposed to a fall hazard of more than 10 feet.
