29 CFR 1910.132(d)(2): The employer did not verify that the required workplace hazard assessment has been performed through a written certification that identifies the workplace evaluated, the person certifying that the evaluation has been performed, the date(s) of the hazard assessment, and, which identifies the document as a certification of hazard assessment: (a) On or about May 4, 2016 at worksite 2707 Barnes, Springfield, Missouri.- employees were exposed to serious injuries in that a written hazard assessment certification had not been completed to certify the PPE needs and requirements for the employees using CHICAGO PRESS BRAKE S/N L19527 in the facility. Abatement certification is required for this item; use the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET for this purpose.
