29 CFR 1910.23(a)(8)(ii): Every floor hole into which persons could accidently walk was not guarded by a floor hole cover of standard strength and construction: On or about May 04, 2016 and at times prior thereto, employees were working on Nomac Drilling Rig #22. On the rig floor, there was a floor opening that did have a cover but it was not stable and secure. The employees were exposed to trip and fall hazards.
