29 CFR 1910.304(b)(3)(ii)(C)(3): Each cord set, attachment cap, plug, and receptacle of cord sets, and any equipment connected by cord and plug, was not visually inspected before each day's use for external defects, such as deformed or missing pins or insulation damage, and for indications of possible internal damage: Drilling Rig 22: On or about October 19, 2016 a portable electrical fan in the mixing trailer had damage to the cord insulation. The cord was not visually inspected from insulation damage, this condition exposed employees to an electrical shock hazard.

29 CFR 1910.305(a)(2)(xi): Cable assemblies and flexible cords and cables were not supported in place at intervals that ensure that they will be protected from physical damage: Drilling Rig 22: On or about October 19, 2016 flexible cords feeding the SCR above the entrance door were not supported and rubbed against the edges of the door when opened, this condition exposed employees to an electrical shock hazard.
