(b) Production floor: On or about April 26, 2016 and at times prior, employees were exposed to caught in hazards when the safety interlock was defeated on the Okuma L250E. (c) Production floor: On or about April 26, 2016 and at times prior, employees were exposed to caught in hazards when the safety interlock was defeated on the Bridgeport GX1000 OSP. (d) Production floor: On or about April 26, 2016 and at times prior, employees were exposed to caught in hazards when the safety interlock was defeated on the LU 400.
