29 CFR 1910.212(a)(1):One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: a) Proof Research Inc., Hand Bedding Area: On or about April 6, 2022 and at times prior, employees were exposed to caught in hazards from the rotating chuck while utilizing the unguarded Genesis bench mounted drill press. b) Proof Research Inc., Grinding/Sanding Department: On or about April 6, 2022 and at times prior, employees were exposed to caught in hazards while conducting sanding operations on the carbon fiber rifle barrels with the unguarded EZ Path by Bridgeport lathe "G1". c) Proof Research Inc., Grinding/Sanding Department: On or about April 6, 2022 and at times prior, employees were exposed to caught in hazards while conducting sanding operations on the carbon fiber rifle barrels with the unguarded EZ Path by Bridgeport lathe "G2". d) Proof Research Inc., Prototyping/Tooling Department: On or about April 6, 2022 and at times prior, employees were exposed to caught in hazards while utilizing the unguarded EZPATH II SD Bridgeport lathe for tooling purposes. e) Proof Research Inc., Prototyping/Tooling Department: On or about April 6, 2022 and at times prior, employees were exposed to caught in hazards while utilizing the unguarded Grizzly G0769 lathe to clean the lock tight from the reaming rods.

29 CFR 1910.212(a)(3)(ii):The point of operation of machines whose operation exposed an employee to injury, was not guarded: a) Proof Research, Chamber/Crown Department: On or about April 6, 2022 and at times prior, employees were exposed to caught in hazards while using items such as, but not limited to: scotch brite to polish the threads on the outer diameter of the rifle barrel. b) Proof Research, Chamber/Crown Department: On or about April 6, 2022 and at times prior, employees were exposed to caught in hazards while using items such as, but not limited to: emery cloth to polish out any rings left from the manufacturing process on the inside body of the chamber.
