29 CFR 1926.1404(c): The A/D director did not review the applicable assembly/disassembly procedures immediately prior to the commencement of assembly/disassembly unless the A/D director understood the procedures and had applied them to the same type and configuration of equipment (including accessories, if any). Location: site On or about 5/12/16, the operator, who was in charge also the assembly director of the Link belt 218 HSL lattice boom crane, had not reviewed the applicable assembly procedures prior to beginning the assembly.

29 CFR 1926.1404(m)(1)(i)(2): The selection of components, and configuration of the equipment, that affect the capacity or safe operation of the equipment was not in accordance with manufacturer instructions, prohibitions, limitations, and specifications. Location: Province Road and route 11 Laconia, NH On or about 5/12/16, a link Belt 218 HSL series crane operating in self assembly mode was not set up in the configuration required by the manufacturer. The supporting pendent was connected to the top picture frame of the base boom where the manufacture states to connect it to the bottom picture frame.
