29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents:    (a) Front porch roof, new residential construction located at 148 Still Pine Bend, Smyrna, GA 30080: On or about May 11, 2016, the employer exposed employees to struck-by hazards as employees operated a pneumatic Hitachi Framing Nail Guns without protective eyewear while engaged in framing activities.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m)  or more above lower levels was not protected by guardrail systems, safety net systems, personal fall arrest systems, or an alternative fall protection measure under another provision of paragraph 1926.501 (b), nor did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502:     (a) Front porch roof, new residential construction located at 148 Still Pine Bend, Smyrna, GA 30080: On or about May 11, 2016, and at times prior, the employer exposed employees to fall hazards of 10-feet, 4-inches from a 4/12 pitched roof, in that personal fall arrest was not provided to employees while engaged in framing activities.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:    (a) New residential construction, front side: On or about May 11, 2016, and at times prior, the employer exposed employees to fall hazards of 10-feet, 4-inches as the ladder provided to access the roof and ground level was not extended at least 3-feet past the landing or secured to the structure.
