29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets. On or about May 24, 2018 at 500 Azalea Park Drive, Holly Springs, Georgia, an employees cutting and passing plywood sheets to the employee working on the roof installing the plywood panels for the roofing deck was exposed to a struck by hazard while working below without a hard hat.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b). On or about May 24, 2018 at 500 Azalea Park Drive, Holly Hills, Georgia, an employee was seen installing plywood panels for decking on the roof without fall protection. The employee was exposed to a fall of approximately 10 feet to the lower level.
