Observed on or about 5/11/16.    b) Jobsite: Scaffold, 8th working platform - An employee conducting window infill activities from a supported tubular welded frame scaffold was exposed to fall hazards, approximately 52 feet above ground level, without the use of an adequate fall protection system.    Observed on or about 5/11/16.      Reiss Masonry Construction, LLC., was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number 1072399, citation number 2, item number 1, and was affirmed as a final order on 2/1/16, with respect to a workplace located at 6th Street & Fairmount Avenue, Philadelphia, PA 19123.
