29 CFR 1910.119(d)(3)(i)(B): Information pertaining to the equipment in the process did not include Piping and Instrumentation Diagrams (P&IDs). Avantor Performance Materials 600 N. Broad Street, Phillipsburg, NJ site - The employer did not ensure that information pertaining to the process equipment was included on the P&IDs: a) Solvents Packaging Auto Fill - Bottles: Filter equipment that was observed in the field for Ethyl Acetate, NMP CAS#872-50-4, PRS-2000, PRS 3000, PRS-1000 and Methylene Chloride were not marked on P&ID 103-029 R10. P&ID 103-029 R10 showed F-01 and F-07 which were removed from the field. b) Acids Anhydrous Ammonia and Ammonium Hydroxide - Relief valve set pressures were marked incorrectly on the P&ID PLT-013. PSV2010 - PSV 2016 were recorded as (????????) on the P&ID when the correct pressure settings should have been 75 psig. Conditions noted on or about 9/8/2016.

29 CFR 1910.119(l)(4): Process safety information required by paragraph (d) of this section was not updated when a change covered by this paragraph resulted in a change in the process safety information. Avantor Performance Materials 600 N. Broad Street, Phillipsburg, NJ site - a) Solvents Packaging Auto Fill - Bottles: Changes to the filter equipment that were observed in the field for Ethyl Acetate, NMP CAS#872-50-4, PRS-2000, PRS 3000, PRS-1000 and Methylene Chloride, were incorrect and did not reflect on P&ID 103-029 R10. The P&ID showed a previous configuration which included F-01 and F-07 which had beene removed from the field. MOCs were developed for these changes however the changes were not reflected on the process safety information. Conditions noted on or about 9/8/2016.
