29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with paragraph (b) or (c) of this section: a) Jobsite: On May 17, 2016 and at times prior thereto an employee performed pipe bursting activities inside a trench box. A plate or other type of shield was not in place on the west end of the trench box to protect the employee from a potential cave-in of the excavated wall, into the end of the trench box. Note) Certification of corrective action IS required for this item.
